Getting Started

At My Flex Disability, we offer in-home services designed to empower you to live life on your terms, in alignment with your unique goals and objectives.
Whether you need specialised care, domestic assistance, social support, or skills training, we have a team of qualified and experienced professionals ready to cater to your specific requirements. Consistency with the same dedicated workers attending to your needs is our top priority.
We are committed to working closely with you to establish a personalised Service Agreement. This document will clearly outline how we will collaborate to provide the tailored support you require.
Next, we’ll arrange your service booking, tailor your support worker preferences, and establish your schedules. If you have a Local Area Coordinator, we’ll coordinate with them and handle all necessary government requirements to kickstart your services with My Flex Disability. Our focus is to simplify the process and ensure you fully understand your options.

How to get started

We believe everyone should have the opportunity to live comfortably in their homes where they feel safe and happy for as long as possible. The following are just some of the at-home care services we currently offer as part of our home care packages in Perth and wider WA:

Get in contact with us
To get in touch with us by calling the corresponding office in Perth, Mandurah or Bunbury below and request to speak with either the Disability Coordinator or the Disability Services Manager.

You can also email our friendly Disability Team at: disability@myflexhealth.com.au

Once our Disability Coordinator has collected all your contact details and specific needs, they will reach out to you or your support person to arrange an in-person meeting at your home. During this meeting, we will thoroughly discuss your requirements and, if relevant, go over your NDIS arrangements.
As an experienced NDIS Provider it’s our job to support you in achieving your goals and strengthening your capacity to live in the community. We provide in-home support for children to adults that are flexible and customised to your personal requirements.
Our Disability Case Coordinator will then work closely with you or your support person to coordinate your admission, develop a Service Agreement, and arrange the necessary services so you can continue to work towards your goals. We will also provide feedback to your Local Area Coordinator or support person as part of this process.
A Service Agreement is a contract that explains important information about the services you have chosen to receive with My Flex Disability and the funding option you have chosen.
After your research and selecting us as your provider, you’re prepared to commence your services. We conduct regular check-ins to ensure your satisfaction and to continuously improve our services to align with your goals and needs.
